Where are you located?

We are located at 2020 North 12th Street. The Clinic is located across the street from Community Hospital and just up the street from Mesa State College.

Where should I park?

There is parking on the side and in the rear of the building. Additionally if needed there is space on Walnut Ave.

What do I need to bring with me to my appointment?

You will be asked to complete a patient information form upon arrival. Please bring your copay (if applicable), current insurance card, any worker's compensation or liability insurance information, a referral from your primary care physician (if applicable), medication list, and any appropriate x-rays or scans previously done (bring the actual scan not just the paper report). Radiology studies that were completed within the local area often times can be view electronically; our staff can help direct you with this information. If you do not have insurance you will be required to pay a $100 deposit at initial appointment; arrangements for payment plans are based on patient needs.

How long will my office visit take?

Please arrive 15 to 20 minutes prior to your appointment to ensure completion of your paperwork and register. Completing the necessary forms in advance will shorten this process. Forms are located on our website www.westernorthosports.com under the patient resource tab.

 

Your appointment allows time to address one priority problem. Your anticipated wait time may be longer than a general practitioner's office because we are a specialty office. The length of your visit depends on the complexity of your case as well as the complexity of others that are being seen that day. We ask that our patients be prepared to spend as much as two hours in our office on the day of their initial appointment. This will allow for enough time for our staff to obtain your past general medical history, assess the history of your current injury or condition, and obtain x-rays if necessary.

How do I get copies of my medical records or x-rays from your office?

If you need copies of your medical records, please call our main number 970-245-0484 and ask for our Medical Records department. If you need copies of your x-rays or MRI scans please call our main number and ask for our radiology department. In accordance with state and federal law, we must have you sign the Authorization to Release Medical Information Form.

I was hurt at work or injured in an accident, what do I need to bring with me to my appointment?

You will need to bring your worker's compensation or third party liability insurance company's name, address, your claim number, and an adjustor or contact person's name and phone number (if applicable).

Will I need to fill out paperwork each time I visit your office?

In order to have the most up to date information on file, our physicians require patients to fill out new paperwork at least once a year. We will ask patients to fill out a new form if they are being seen for a new ailment even if they have been seen in the past year.

What happens if I need surgery?

If you have an orthopedic problem that requires surgery, your physician will explain your options and answer any questions or concerns you may have. Our staff will assist you in scheduling and pre-certifying your surgery.

What forms of payment do you accept?

For your convenience we accept cash, check, debit card, VISA, MasterCard, or Discover, and Care Credit.

What do I do in case of an emergency?

Our phones are answered by our office staff from 8:00 a.m. to 5:00 p.m. Monday through Friday. If a medical emergency arises after hours please call our office. Emergency calls are directed to an answering service and messages will be transmitted to the doctor on call.

We have an injury clinic on Saturday from 10:00am to 11:00am while District 51 school system is in session.

What if I can't pay my bill in full?

Please call our main number, 970-245-0484 and ask to speak with a billing specialist. We will be happy to assist you with your insurance billing and setting up a payment plan.
