Insurance Information
We will file all insurance claims. We accept assignment on Medicare, HMO and Blue Cross Blue Shield and some PPO's. Even though you have insurance, you are ultimately responsible for your charges.
Cancellation
If it is necessary to change or cancel an appointment, we request at least one day's notice.
Telephone Calls & Emergencies
During Office Hours: Our personnel are well trained to answer most of your questions. If they are unable to help you, the doctor will return your call as soon as possible.
After Office Hours: Our phone is answered 24 hours a day; a doctor is always on call. In case of an emergency, go to the emergency room of the hospital of your choice for care and have the E.R. contact the physician on call.
Payments
Payment is due for office services at the time they are rendered, unless prearrangements are made with the office. After payment is made we will submit the claim to your insurance company for reimbursement. Please keep a copy for your records in the event you would have to resubmit your reimbursement. We offer a 10% discount if payment is made at the time of service. If at any time the payment at the time of service becomes a burden to you, please discuss the matter with our business office in advance.
Prescriptions & Refills
Prescriptions and refills are issued during office hours only. During evenings and weekends, it is difficult for the physician to determine if a prescription or refill is indicated when they don't have your medical records.
